Sacred Heart Catholic welcomes all students who wish to apply for admission regardless of race, religion, gender, or national origin. Enrollment is open for the 2025-2026 school year. Please see the information below.


FACTS Management will be administering our 2025-2026 school registration, tuition, payment plans, and financial aid for our school families, beginning with the $100 registration fee. Please click the logo to begin.

K-8 Registration Materials for 2025-2026:


We are excited about the upcoming 2025-2026 school year and hope you are too. Below are the forms that need to be completed for students in grades K-8 for the school year, which starts on August 6, 2025. 


Sacred Heart Catholic is accepting K-8 students for the following grades, based on current pre-enrollment:

  • Limited openings:  Kindergarten, 2nd Grade, 3rd Grade, 6th Grade
  • Multiple openings:  7th grade, 8th Grade
  • Waiting List:  1st Grade, 4th Grade, 5th Grade

If you are interested in enrolling a child in Grades 1, 4, and/or 5, please complete the Waiting List Request form.


TO SECURE A SPOT FOR THE NEXT SCHOOL YEAR:

  1. Complete the Student/Family Information Form (In Registration Forms PDF: see below) and submit Non-Refundable Fee in your FACTS account. Completion of form will indicate interest, and you will be notified of acceptance. Once accepted:
  2. Register with FACTS Management. Required of all families.
  3. Non-refundable $100 Registration Fee will be deducted from your FACTS account on June 25, 2025.
  4. The $125 K-8 Technology Fee will be deducted July 20, 2025.
  5. Complete the Financial Aid Application to be considered for any scholarships and/or financial aid. Recommended for all families. 


Forms found in the Registration Forms PDF require a printer or Adobe editing program. Please scan or save completed forms and email to info@sacredheartschool.us. If you do not have access to a printer or Adobe, please contact the office at 812.283.3123 to pick up the forms.


INFORMATION DUE BY AUG. 1, 2024   

  • Read the School Handbook and the Technology Responsible Use Policy and then sign and return the Signature Document agreeing to abide by both these documents. Required yearly from all families. Not available until Summer 2025.
  • Sign Consent to Services document from St. Elizabeth Catholic Charities. (In Registration Forms PDF) This allows your child to participate in presentations or activities the counselor may conduct with whole classes, small groups, or one-on-one. Parents will be contacted if the counselor thinks their child would benefit from ongoing or one-on-one counseling. Required yearly from all families
  • Race and Ethnicity of Student Data - Home Language Survey. (In Registration Forms PDF) To be completed once, at the first Indiana school the student attends. 
  • Submit a copy of each student's birth certificate
  • Submit a copy of the student's baptismal certificate, if baptized Catholic
  • Immunization form from the student's physician for students in Child Care Ministry, Preschool, Kindergarten, 6th grade, and new students and/or after any new vaccinations have been completed. (In Registration Forms PDF)
  • Medication Form only if a student is to be given any medicine at all during the school day, prescribed, or over the counter. (In Registration Forms PDF)
  • Sacred Heart Catholic Aftercare program provides after school care from 3:00 p.m.-6:00 p.m. Please register here by Aug. 1, 2025.
